You're not alone. We can help.

Our professional social workers and nurses make home visits, evaluate your loved one's needs, and help arrange for the best care to maintain their safety, independence and dignity.

How do we begin?

We start by carefully evaluating the needs of the elderly or disabled person. After a comprehensive in-home assessment of the individual's physical, mental and social functioning, we prepare a personalized care plan. The care plan addresses both short-term and long-term solutions.

What happens next?

We can help you choose, arrange and monitor care, including:

  • Quality In-home Caregivers (hourly or live-in): We will help you arrange for the most caring, reliable and capable in-home help, carefully chosen to match the needs and personality of the client.

  • Assistance with Moving: We have the first-hand knowledge to help you choose the best Senior Community, Assisted Living, Rehabilitation Center or Nursing Home, assist with the move, and provide support during the transition.

  • Coordination of Care: Our professional staff can coordinate all aspects of care, including, for example, escort to medical appointments, medication management, skilled home nursing care, physical therapy, social and recreational activities, adult day care, grocery or meal delivery, transportation, home maintenance, bookkeeping, medical claims coordination, and friendly visiting.

  • Expert Professional Referrals: You want the finest care for your loved one. We can recommend physicians, geriatricians and other medical specialists, as well as elderlaw attorneys and financial planners to help you make the best decisions.
Will my loved one's care be monitored?

Frequent, regular visits by the professional care managers lead to quality of care for your aging and/or disabled relative. Ongoing communication with physicians and families ensures that the client's needs are being met.

We can provide crisis intervention if needed, and act as a liaison to long-distance families. Because we are here to serve you, we can be flexible and responsive. We also are available to coordinate care for Guardianships and Trusts.
